As you can see here the muscle memory of looking down at the airway rather than the video screen in front of me is apparent. It is a totally different technique that took some getting used to. But as a mentor told me, you should become proficient at all intubation techniques because you never know when someone will hand you an unfamiliar blade. In this scenario the locums doc encouraged the glidoscope because Covid transmission was still concerning, so his strategy was encouraging physical distance from the patient’s face. Look at the exhalation phase of the flow graphic. The volume does not return to baseline 0. This means the pt has inherent PEEP, aka Autopeep. The compliance is worse because now, not only can the lungs not blow off air, they cannot receive air as well. If a balloon is 3/4 of the way full it can only take so much more before it pops. Most vents have a pressure limit so as a result of limiting air pushed in the TV will start to drop which will eventually worsen acidosis. This happened because of the two changes noted at the bottom of the screen. The RR was increased from 9 to 16 and the iTime was increased from 0.8 to 1. This was likely done to “correct” acidosis. The immediate result is a drop in ETCO2 but eventually it will worsen acidosis because of the above pressure limit. It will also worsen vent asynchrony as the pt will be very uncomfortable. Their venous return is impeded and blood pressure will drop. If the PIP + the Plateau were high the issue is poor compliance (from within the lung i.e PNA, ARDS, atelectasis, etc.). Also note the high ETCO2, the looooong exhalation phase, and the low/norm respiratory rate of 9, the lower volumes (~4ml/kg). graduated NP school in 2016. It was the culmination of my life’s greatest challenge. It has been the best decision I ever made. Would I have made it sooner? No, because I believe we all have a journey and without the right experiences the foundation for success isn’t properly laid. I was a part-time bedside nurse for 15 years while my husband traveled for work. It was the best of both worlds - SAHM + nursing. As a serial job hopper, I’ve had a lot of experiences.
